Het CIZ werd per 1 januari 2015 onder de Wet langdurige zorg een publiekrechtelijk ZBO. De organisatie was in 2005 opgericht als stichting die landelijk de indicatiestelling voor de AWBZ overnam van regionale indicatieorganen.

Het Centrum indicatiestelling zorg (CIZ) beoordeelt onafhankelijk of mensen recht hebben op zorg vanuit de Wet langdurige zorg (Wlz), bijvoorbeeld voor verpleeghuiszorg of zorg voor mensen met een verstandelijke beperking. Het CIZ beoordeelt jaarlijks honderdduizenden aanvragen en werkt vanuit meerdere regiokantoren. Het bureau is een zelfstandig bestuursorgaan onder het ministerie van VWS.
